Journal 3 - Essay Example Instead of telling us what we needed in the bag, the teacher gave us suggestions about what we might need and invited us to come up with ideas for what we would like to pack. After we finished, we discussed why we chose the items we did. I selected this activity because it gives a good example of how my teacher performed more of a helper/ mentor role with us, rather than a strict lecturer role. Due to her encouraging way of doing the lesson , there were fewer arguments than there would have been if she had required all of the students to put a predetermined list of items into the suitcase. I learned that being a helper or a mentor to children can be a smoother and more effective way to assist in the learning process of children. If I were to do this activity with my children, I would discuss with them before we began about what they might place in their suitcases. After researching these sites, I found that these sites support much more than socializing amongst friends. It has become one of the most effective ways for businesses to communicate and build relationships with other business people and potential employees. Social networking has provided businesses with the following: business development or sales, raising business capital, professional development, getting job assistance with others in your company, recruiting board members, management of employees and social or personal reasons.[iii] Social Networking can add value to a company externally as well as internally. It can be a very powerful tool and when utilized properly we can learn from others as well as ourselves. There are many companies that have taken the idea of these social sites and utilized them internally. They are using social networking as marketing tool for growing their business. Social Networking is now essential professional competency for employees at all levels. Professionals need to know how to gain visibility and credibility in their target markets and how to build and maintain relationships for long-term growth. They need to develop strategic networking skills and practices to excel. It is the most important tool for intelligence gathering. In a business setting, such as conferences, meetings, and outings, people need to find the latest information on resources, trends and best practices. It is a critical strategy for business development. Employees can create a network of profiles and connect with one another. Enabling connections among employees, the company could easily establish virtual teams, bring new employees up to speed, improve collaboration and increase retention among people who hadn’t felt a strong sense of belonging.[iv] Social Networking is a method for personal retention because it creates feelings of inclusion and helps people from diverse backgrounds feel listened to and valued. Building up a network of diverse people is very helpful for any business. Contacts with different clusters or groups would help your possibility of greater opportunities. A closed group of contacts will not allow you this kind of exposure. Networking can help you with your contact list. This would then help you to expand your sales and social base. [v] It remains the primary technique that people use to find jobs, change careers or land on their feet after a layoff, merger, or reorganization.[vi] Through the company’s social network you can create a word-of-mouth marketing stream that reaches people you would never be able to reach on your own or other means. Experts can provide feedback or validation regarding your current ideas or help to create new ones. You can create an advisory board of business people that are regularly available to you and committed to the growth and success of your business. People are given the opportunity to brainstorm which allows you to get outside of the normal way of thinking and create new and innovative ways to grow your business. People in your social network can connect you with potential employees.[vii] Companies are communicating and connecting using external sites as a tool to check up on employees who are working for them or are a potential candidate. We want to make sure we are ahead of the game, the who’s who if you will. Potential employees seem to be younger and younger. They are not all exploiting the typical Monster.com and CareerBuilder.com to post their resumes or look for jobs anymore. They are developing personal sites with LinkedIn.com and Facebook.com, which are allowing a more personal view of a potential employee or client. More than one in five employers searches social networking sites to screen candidates. There are many areas of concern that can be researched on networking sites including: information about drug and alcohol use, inappropriate photos, poor communication skills, bad-mouthing of former employers or fellow employees, inaccurate qualifications, unprofessional screen names, notes showing links to criminal behavior, and confidential information about past employers. This is a very effective way to help determine whether a candidate should be hired. You want an employee that can show a professional image, solid references and are a well-rounded candidate in terms of their skills, accomplishments and overall fit for the company.[viii] Closing Social Networking is a smart and strategic way to keep our company connected and communicating both internally and externally. Analysis of Letter from Birmingham by Martin Luther King Jr. Martin Luther King Jr., is one of the most recognized, if not the greatest civil rights activist in this century. He has written papers and given speeches on the civil rights movement, but one piece stands out as one of his best writings. â€Å"Letter from Birmingham† was an intriguing letter written by King in jail in the city of Birmingham, Alabama. He was responding to a letter written by eight Alabama Clergyman that was published in a Birmingham Alabama newspaper in 1963 regarding the demonstrations that were occurring to stop segregation. The intended audience for this letter was of course the eight clergymen, but he also had a wider audience in mind because instead of sending each individual man a letter he had it published in the local newspaper. In his letter, King starts by addressing the clergyman with â€Å"My Dear Fellow Clergyman† as if to put the men at ease and keep them open-minded. In the first paragraph King states that he does not usually respond to such correspondence, but compliments them by saying that since they are genuinely good men, then he would respond to their criticisms by writing this letter. In the second paragraph, he addresses the â€Å"outsider† issue. In the letter written by the clergyman they say (like it was something new), that they were now facing demonstrations led by outsiders (King). An Inspector calls The inspector has many functions in 'an inspector calls', the inspector is the centre point of the play, and the main events of the play rotate around him. He controls the entrances and exits of the play. He also has a grip on the topic of conversation, and is not afraid to anything and has the confidence to interrupt to re-tighten his grip on the conversation. The inspector also develops the topic of conversation from person to person in a methodical fashion. The inspector controls the development of events, who will speak and when they will speak, who will leave and who won't leave, who sees the picture of Eva smith and who won't. Even when Priestley describes him, when he first appears on stage, he is described in the terms of 'massiveness, solidity ad purposefulness', significantly showing that he is unstoppable, and plays a great part in the play. He has a 'habit of looking hard at the person he addresses before speaking' gives the audience an impression of how he gets down into the truth. His role in the play isn't the role of a conventional inspector, which is to confront each character with the truth, but he wants the Birling family to morally condemn what they have done. He works in a methodical, chronological order, moving from the start to end, of eva smiths journey through life involving the Birling family, from character to character, this is because he acknowledges 'otherwise there is a muddle' and if the character are given the chance, to be confronted by the truth they will try and defend themselves, and get themselves out of that situation, in order not to accept the truth. The inspector has arrived just after Mr.Birling has been discussing his views on life, co... ... intimidating line from a lower social status inspector to a higher classed business man. The inspector doesn't 'have much time' to procrastinate questions, so he will just cut in and ask them. This helps the inspector to get through every person in a methodical logical order; hence the plot can be developed. From the following you can draw the conclusion that the inspector plays a massive role in the inspector calls, from a dramatic instrument to have narrative functions, and acting as a vehicle to convey Priestley's socialist views. If the inspector wasn't in the play, the play wouldn't work; life would be difficult for Priestley to put across his views. The essential elements of the play to make it interesting are lost if the inspector wasn't there, so he is very essential, and is one of the most important characters of the play 'an inspector calls'.

This will ceaselessly be a question challenging the intentions of Shakespeare’s literature. However, [didn’t Wittlin say don’t start with however else its after a semi-colon] the question in this revision of morality in Shakespeare’s Henry IV, Part one is, is there even a moral center in Shakespeare’s Henry IV, Part one? Humanity is incapable of absolute goodness; therefore, there is no moral centre in Henry IV, Part one since the three major characters, King Henry, Prince Hal, and Sir John Falstaff, are all somewhat morally flawed. Shakespeare reveals the imperfection of human nature through the behaviour of his [these] characters. First of all, King Henry sets a presumed reputation as the religious, loved and strong leader of England in Henry IV, Part one for his subjects. However, his supposed virtues are only results of his concealed faults. Ironically, the King can be quite blasphemous, despicable, and pathetic. Throughout the play, Henry is evidently repenting for his conduct in his acquirement of the British thrown. This is shown in his belief of â€Å"whether God will have it so, /†¦To punish my [King Henry’s] mistreadings† (III. ii. 4-11) and that â€Å"God pardon† (III. iii. 29) Hal for his unpunished sins of his bad company. Furthermore, King Henry’s disgraceful conduct clearly reveals the false reception of love from his subjects. This is especially revealed in his relationship based on conditional love with Prince Hal. His opinion of Hal, which changed from a state of â€Å"riot and dishonour† (I. i. 4) to one of â€Å"charge and sovereign† (III. iii. 161), is only established on restricted affection and Hal’s social image, rather than a personal benevolence between parent and child. In addition, another of King Henry’s loathsome features is again shown through his attainment of the throne: deceitful behaviour. Furthermore, the King’s pathetic nature is revealed by his insecurity. The play begins with the King expressing his paranoid worries, being â€Å"so shaken† and â€Å"wan with care† (I. i. 1-2), accordingly presenting the audience with its first impression of the supposedly strong leader. King Henry deceitfully attempts to use the â€Å"chase† of the â€Å"pagans in these holy fields/†¦for our [England’s] advantage† (I. i. 24-27) to distract the â€Å"civil butchery† (I. i. 13) back home in England. Overall, the life events of King Henry IV’s does not present a very moral reputation for a man of worthy of such power and prestige. Secondly, Prince Hal clearly shows both positive aspects and negative aspects, as his character undergoes great change in Henry IV, Part one. Hal gives the audience the impression of his intentions to â€Å"throw off† (I. iii. 05) his uncouthly behaviour moral to please the King, the alleged victim in Henry IV, Part one. Hal believes he can â€Å"find pardon on† his â€Å"true submission† (III. ii. 28) by satisfy his father’s expectations for the throne’s heir and discard the values of his loving surrogate father, Falstaff. As revealed in the previous quotation of pard oned submission, one of Hal’s admirable aspects is his open ability to accept his faults; however, it seems his judgement regarding the class, justice, and honour system remains stereotyped by knightly tradition. Although Prince Hal’s resultant persona is traditionally considered positive, Hal’s most commonly shown qualities in the play are characterized as manipulative, superficial and unemotional, all of which further reveal his immoral faults. Hal’s manipulative nature is exposed throughout Henry IV, Part one. Prince Hal’s manipulative intelligence is first revealed in his soliloquy, where he vows to â€Å"falsify men’s hopes/ and†¦so offend to make offense a skill† (I. iii. 205-211). Hal’s aptitude for manipulating is further proven in his sudden abandonment of Falstaff and his low class company, as foreshadowed when Hal symbolically states that â€Å"by breaking through the foul and ugly mists†¦my [Hal’s] reformation†¦shall show more goodly† (I. ii. 196-). In this quote, the clouds represent Falstaff and company and the beauty in reference [to†¦] is the reformed Hal. An addition to Hal’s [im] amoral traits is his superficiality. Hal’s superficiality is shown in his judgement of physical image. This is shown in his constant vulgar references to Falstaff’s obesity: a â€Å"fat-witted with drinking of old sack† (I. ii. 2) and his abandonment of Falstaff’s role in his life after his reformation. Hal’s commitment [to] the traditional expectations of honour results in the betrayal of Falstaff’s hedonistic approach on life and his only endeavour is to please the man who had offered a pitiful excuse of love incomparable to what Falstaff had to offer: unconditional love. These examples of Hal’s superficiality also support Hal’s lack of sympathy for others. Hal’s cold behaviour towards others is shown in his hypocritical approach for Falstaff’s hedonistic [maybe use self-gratifying] attitude. Hal is unaware of his own form of intemperance: he strives to improve his own self-image at the expense of others. Despite Hal’s admirable traits as a respectable member of court, as a human being, Prince Hal’s amorality is quite apparent by the distinction of his actions. Lastly, despite Sir John Falstaff’s self-gratifying lifestyle, he seems to be the most moral character in Henry IV, Part one, although not wholly moral because as previously addressed, human nature is inept of utter goodness. Due to Sir John Falstaff’s philosophies, many have claimed to be fond of his self-indulging ways but admit the ridicule behind paying formal respect to such a person. Falstaff cleverly manipulates others for his own welfare; however, it is only in good nature. This is proven in Act III scene iii, when Falstaff distorts the situation of his debt to Mistress Quickly into one of an accusation of her being the thief of his â€Å"picked†¦pocket† [wasn’t he really pick pocketed? ](III. iii. 53), and more wittingly forgives her in the end as she goes to prepare his meal, intending no spite upon the hostess. Falstaff deceives, cowards [not an action; cannot be used in this senctense], drinks â€Å"of old sack† (I. ii. 2) and commits virtually every sin. Shakespeare masterfully moulds these negative aspects into unusual forms of virtue in Falstaff’s character by showing that Falstaff means no harm. In doing this, Shakespeare cleverly twists the faults upon the regal members of society by building the play upon the disputes between themselves; thus, showing the power of such subtle issues, barely considered sinful, causing â€Å"civil butchery† (I. i. 13), whereas the sinful ways of â€Å"Old Jack Falstaff† (II. iv. 72) has no such effect. Although Falstaff’s pleasure priorities may be rather farfetched, his â€Å"gift†¦is youthful irresponsibility, which must be cherished even though it cannot last† (p. xx). Falstaff’s commonly repeated idea that â€Å"young men must live† (II. ii. 90) emphasizes his belief in the value of youth ful irresponsibility and luxury. Shakespeare grants Falstaff the embodiment of human nature itself, excluding extreme wicked sins, leaving Falstaff’s childlike benevolence untouched; this is shown as he pompously states, â€Å"I have more flesh than another man, and therefore more frailty† (III. ii. 167-169). Falstaff serves as a bringer of human nature as he serves to foil all other characters therefore revealing everyone moral flaws yet remaining the most moral character due to his youthful benevolence. In conclusion, Shakespeare brilliantly provokes the audiences’ involvement in his plays by presenting them with intellectual trials to the mysteries of life. Faithful, Firm and True Book Critique Free Online Research Papers Dr. Titus Brown is a History professor at Florida AM University located in Tallahassee, Florida. He is a graduate of Albany State University in Albany, Georgia. He is the author of many books including the wonderful Faithful, Firm and True. Faithful, Firm and True gives the readers a detailed look at how formal and organized education was created in the South for free blacks. Assuming Dr. Brown did extensive research to basically travel back in the past to gain knowledge on the inception of something so historical and life changing in society. The question being answered here is how was an education system formed for African Americans, what were the trials and tribulations of the establishment for formal education for African Americans and what was the success of it. And Dr. Brown answered these questions thoroughly in Faithful, Firm and True highlighting the education system in Macon, Georgia and the establishment of Lewis and Ballard High Schools. To illustrate the foundation of formal and organized education for African Americans in the South, Dr. Brown presents the Freedmen’s Bureau and its efforts, accomplishments and failures. In the book, Dr. Brown provides recollections of individuals who were educators during this time. The era is during the Reconstruction, post-Civil War. He mentions one educator’s experience, William Cole. According to Dr. Brown, â€Å"Cole and other teachers were promised pay of twenty-five dollars per month plus a special allowance of thirty cents per day for rations. After working for four months, Cole wrote Superintendent Eberhart complaining that the teachers â€Å"have received but two months pay without the stipulated amount for rations†.† Dr. Brown proceeds to add Cole stating that, â€Å"Parents of the students paid nothing, â€Å"as they were not able to pay to have their children taught.† Moreover, inadequate funds prevented the school from obtaining unif orm textbooks for students.† Lastly Dr. Brown lets it be known that â€Å"Cole’s class purchased its own books at a â€Å"very high price†.† This was merely one the problems that posed during this period. White men ran the country and not much effort was put forward in the education of blacks. It was a system of oppression, one we still see today. To obtain work for money, one most likely had to be educated, with no resources for education, one was left uneducated and unable to obtain work, so they most likely had to return a pseudo form of slavery in order to survive after being legally emancipated. Dr. Brown makes it clear that it was a struggle for blacks to obtain education. The reader learns that many schools geared for African American education were in the basements of churches and most likely solely funded by clergy and church members who had children enrolled. The Freedmen’s Bureau and Freedmen’s Aid Commission alike were enacted for the advancements of African Americans. They worked hard at sticking to their word but as one can read, they often failed. The Freedmen’s Bureau and other organizations were typically ran by Republican politicians but once terrorist groups like the KKK rose to power and started terrorizing individuals like those associated with the Freedmen’s Bureau and African Americans, they began to back away. Eventually, these initiatives were closed down and people of power backed out assuming there will be no end to the violence and opposition basically leaving the individuals they intended to help to fend for themselves. This brought upon the establishment of the AMA, the American Missionary Association. The AMA used the four schools already established in Macon and saw that additional schools were opened such as those in the outbuilding in the Lincoln Home Yard and a room in the Freedmenâ₠¬â„¢s Hospital. The AMA also oversaw the proposal of the first school building in Macon, Georgia. Enrollment dropped when a tuition fee was announced but the individuals in the system raised $1,500 to help and enrollment rose. The school system endured many rise and falls in its establishment but that never stopped production. Dr. Brown’s argument of the hardships that were endured in the establishment of the education system of African Americans is valid and persuasive, in the sense that reader was thoroughly educated and able to understand and maybe empathize with this struggle and the end feeling of accomplishment. In the end we see clearly that Ballard students were well educated and advanced on to other positions, mostly of leadership and also giving back by educating others as they were educated. The reader is able to grasp something so simple as, Hard Work does pay off. Aim: We aimed to analyze alterations in the morphology and cytomorphometry of buccal epithelial cells of Type 2 Diabetes Mellitus patients using oral exfoliative cytology technique and determine its importance in public health screening, diagnosis and monitoring of diabetes mellitus. Material Methods: The study was carried out in 100 Type 2 Diabetes Mellitus patients and 30 healthy individuals. Smears were taken from the right buccal mucosa and stained by the Papanicolaou technique. Staining with Acridine orange was carried out to view qualitative changes with confocal laser scanning microscope (LSM-510 Meta).The cytomorphometry was evaluated using IMAGE PRO PLUS 5.5 software with Evolution L.C camera. All findings were statistically analyzed. Introduction Diabetes mellitus (DM) is not a single disease entity but rather a group of metabolic disorders sharing the common underlying feature of hyperglycemia. Hyperglycemia results from defects in insulin secretion, insulin action or both. The chronic hyperglycemia and resultant metabolic deregulation of carbohydrate, fat and protein may be associated with secondary damage in multiple organ systems, especially the kidneys, eyes, nerves and blood vessels.1Recent estimates indicate that the number of people with DM in 2010 is 285 million and is expected to be 438 million in 2030. The global prevalence rate is 6.6% in 2010 and is expected to rise to 7.8% in 2030.2 India leads the world with largest number of diabetic subjects earning the dubious distinction of being termed the â€Å"diabetes capital of the world†. According to the Diabetes Atlas 2009 published by the International Diabetes Federation, the total number of people with diabetes in India is 50.8 million and is expected to rise to 87 million in 2030.3 It can no longer be considered a disease of affluent nation alone, it has become a global problem, a major epidemic of the twentieth century, and one which shows no sign of abating. 3 DM is the leading cause of end-stage renal disease, adult-onset blindness and traumatic lower extremity amputations. The prevalence of DM is increasing sharply in the developing world as more people adopt a sedentary lifestyle, with India and China being the largest contributors to the world’s diabetic load.2 Oral Exfoliative cytology is a relatively simple and non-invasive clinical technique which has the potential to be developed as a routine investigation for screening of DM. It can be used chair-side during routine dental examination.4The various alterations in the cytomorphology of the oral mucosa in diabetes and characterization of these changes give clinicians a more accurate image of what really happens during diabetes. So in our study, the cytomorphometric and cytomorphologic changes were compared according to the different glucose levels of diabetes. In addition to light microscope, we used confocal laser scanning microscope, to ascertain the various cytomorphologic changes seen under light microscope. It is possible that the degree of cellular change depends on the progression of diabetes. Hence the present study was undertaken as this research area has received little attention to date. Materials Methods Selection of subjects: The study was carried out in Type 2 DM patients attending the diabetic outpatient department of General Medicine of Sri Ramachandra University, Chennai, India. These patients were under regular monitoring of blood sugar levels and subsequent treatment. The experimental group included 100 Type 2 DM patients and the control group consisted of 30 healthy individuals free of any systemic diseases with clinically normal oral mucosa. Study groups: The entire study sample was grouped for statistical analysis, based on the recent Fasting Plasma Glucose (FPG) levels as follows: Group I: FPG 110-150mg/dl; Group II: FPG 150-200mg/dl; Group III: FPG >200mg/dl; Control: FPGExclusion criteria: Individuals with habit of tobacco use in any form, habitual alcohol intake, any other systemic illness, clinically evident nutritional deficiencies like anemia, presence of oral sepsis were excluded from the study. After selection of the patient, informed consent was obtained and the p rocedure was carried out. In addition, the biochemical hematological measurements were carried out to exclude anemia other systemic disease. Smear collection and preparation: Patients were asked to rinse their mouth to remove any debris. Following this, with a gentle scraping motion, cells were scraped from clinically normal appearing right buccal mucosa. The scrapings were then evenly smeared onto the glass slide and immersed in 95% isopropyl alcohol in a coplin jar, for half an hour. Staining technique: Smears from all the samples were stained by the Papanicolaou technique (PAP).For few samples two smears were taken from the same site, one stained using PAP and other using Acridine orange(AO), a fluorescent dye, to view with confocal laser scanning microscope. In cases with very high plasma glucose levels smear was stained with Periodic Acid Schiff (PAS) technique to assess presence of candida. Cytomorphometric assessment: The cytomorphometric analysis was done using IMAGE PRO PLUS 5.5 software with Evolution L.C camera. In each of the PAP stained slide, ten fields were chosen by systematic sampling in a step wise manner, moving from left to right and then down and across in order to avoid measuring the same cells again. Cells with clearly defined cellular outlines were only chosen those that were clumped, overlapped or folded were excluded for analysis. The cells were projected on to the monitor via the camera at 40x magnification and images were captured. In the software main menu, the function ‘Measurement mode’ was selected and the icon specifying ‘polygon’ was enabled to analyze the area of interest. The nuclear area (NA) and cytoplasmic area (CA) were obtained by drawing around the nuclear and cell boundaries using the cursor. The areas were recorded in square microns. The nuclear cytoplasmic ratio (N/C) was calculated for all the cells. The various parameters calculated were by the Image analysis software thereby reducing the subjective error. Cytomorphologic assessment: The PAP PAS stained slides were viewed under light microscope whereas the AO stained slides were viewed under confocal laser scanning microscope (LSM-510 meta).Morphologic assessment included inflammatory component, cell degeneration, micronuclei, binucleation, intracytoplasmic inclusion, candida, keratinization, intracytoplasmic microorganisms any other changes. The nuclear size can therefore be altered by change in the content of DNA or proteins. There is usually twice as much protein as DNA in a nucleus.13 Hyperglycemia induces a compensatory increase in insulin secretion which in turn causes increase in protein formation. Insulin has several mitogenic functions, including initiation of DNA synthesis in certain cells. These may account for the i ncrease in NA seen in diabetes patients.14 Our study revealed a decrease in CA with increase in blood sugar level i.e.GroupIII mean CA was 2270.63  µm 2 and control mean CA was 2771.6  µm2 (Figure1). This finding is contradictory to studies by Alberti et al 5 and Shareef et al6 who found that CA did not show any significant difference in diabetics and Hassan et al7 who reported a significant increase in CA. However this finding concurs with Prasad et al15 who reported increase in cell diameter and cytoplasmic diameter with increase in glycemic status. Ogden et al10 have reported a similar reduction in cell diameter in patients with habit of alcoholism. The reduction in CA in our study could be due to the dehydrated condition of the diabetics. Increased blood glucose causes dehydration, polyuria polydipsia, intracellular extracellular dehydration. Glucose does not diffuse easily through the pores of the cell membrane causing an increase in osmotic pressure. This increase in osmotic pressure in extracellular fluid causes o smotic transfer of water out of the cells explaining the reduction in CA.16 The comparison of N/C ratio between control and experiment groups showed a steady increase in N/C ratio (from Control-0.0188 to Group III-0.0369) with increasing blood glucose levels(Figure1) .This may be due to a real increase in NA and decrease in CA in diabetic patients, rather than just a deviation from normal. This finding concurs with Rivera et al17 and Prasad et al15 who also reported increase in N/C ratio. In the qualitative changes we found that micronuclei (MN) was present in 44 cases which was statistically significant (p=0.018). MN are known biomarkers of genome damage and has been studied in buccal cell systems. They provide a convenient and reliable index of both chromosome breakage and chromosome loss.MN is found in cells that have completed nuclear division.MN was judged according to criteria by Tolbert et al. 18 Another biomarker of genome damage is nuclear bud formation which was also visualized but not statistically analyzed.16 The nuclear bud has the same morphology and staining properties as the nucleus, however, its diameter may range from a half to a quarter of that of the main nucleus. Hyperglycemia results in formation of Advanced Glycation End products (AGE), endothelial nitric oxide synthase uncoupling, activation of protein kinase C activation of polyol pathway. This in turn causes activation of reactive oxygen species which results in induction of oxidative stress . Oxidative stress is an imbalance between the production of reactive oxygen species and the biological systems ability to readily detoxify the reactive intermediates or to repair the resulting damage. The effects of oxidative stress depend upon the size of these changes, wherein a cell can overcome small perturbations and regain its original state. However, more severe oxidative stress can cause cell death and even moderate oxidation can trigger apoptosis, while more intense stresses may cause necrosis. Oxidative stress has been found to cause genomic damage in DM. 19 This supports the observation of MN in diabetic patients. Few other studies also showed increase in MN in lymphocyte cultures of type 2 diabetics.20, 21 This is the first reported study in which an attempt has been made to study qualitative changes of buccal smear using confocal microscopy. The use of fluorescence dye (Acridine Orange) and visualization under confocal laser scanning microscope enhanced the demonstration of nuclei and micronuclei. They also helped to avoid false positive results. Statistical analysis revealed that cell degeneration was present in 44 cases out of 100 cases. It was statistically significant (p=0.05) implying that increase in blood glucose level leads to oxidative stress and showed increase in degeneration and necrosis. The various qualitative changes included karyorrhexis, pyknosis, apoptosis chromatin abnormalities. Previous studies by Alberti et al5 Shareef et al6 also demonstrated qualitative changes of karyorrhexis. Another significant qualitative change found was the presence of keratinization. This finding is in accordance with an earlier study by Zimmermann et al22 who stated that endocrine dis orders like DM had increased keratinized cell count in the buccal mucosa. The increase in this keratinization may be a compensation for decreased salivary flow.5 Binucleation was present in 45 cases out of 100 but showed no statistically significant result. The significance of these binucleate cells is unknown, but they are probably indicative of failed cytokinesis following the last nuclear division in the basal cell layer. Studies by Alberti et al5 and Shareef et al6 also showed similar findings of binucleation. Increased number of polymorphonuclear leukocytes was observed in the experiment group than chronic inflammatory cells. Increase in inflammation could be due to decreased salivary flow found in diabetics owing to hypofunction of the salivary glands. Hassan et al7 in their study found that incidence of inflammation was higher in diabetic smears. Figure2 Figure3 shows the various qualitative changes viewed under light microscope and confocal microscope. Conclusion: From our study, we could conclude that diabetes produces significant alterations in the cytomorphometry and cytomorphology of buccal epithelial cells. The use of confocal laser scanning microscopy demonstrated qualitative changes with higher resolution and clarity and helped in reducing the possibility of recording false positive and/or false negative observations. Clinical significance: Exfoliative cytology is helpful in diabetic patients who have aversion and fear to needle pricks as it is painless and can be carried out regularly .The minimal time and ease of the procedure is beneficial in mass screening and public health awareness programme. They also aid in monitoring of diabetes mellitus throughout the lifetime of the patient, thereby decreasing the morbidity and preventing long term complications. I have understood that for the success of any program, it is imperative to allocate adequate time to ensure each and every activity is effectively undertaken. In this program, the major activities included training on time management, counseling, training on the entrepreneurial skills, communication skills and group mentoring. To ensure that the p articipants will have adequate time to learn the various skills as outlined on the time table, I have indicated the time and the occurrence of the activities. In the same way, I have acquired skills on how to prepare a work plan that involves the date of the commencement of the activities as well as the deadline and the occurrence of the activities. This is a very crucial skill that I will put into practice during my future careers as a project manager. The importance of program evaluation In the process of preparing the program proposal, my skills of program evaluation have been enhanced. Most importantly, I have gained an extensive knowledge on how to implement logic model during evaluating my future program as an administrator (McLaughlin and Jordan, 1999).
